Sunflower Health Plan wants to understand your preferred method of record collection for the upcoming HEDIS hybrid season. We recognize that chart chase season can be a busy time for your office, and our goal is to make the upcoming season as effortless as possible.

We’re collecting and recording our providers’ preferences and would like to know yours!

Please visit our Chart Retrieval Preferences Form to fill out your service location information, preferred retrieval method and copy service vendor information. Alternatively, you can download the form (PDF) and return it by email or by faxing to 855-475-4399.
