Network Enrollments and Updates
Welcome to the Sunflower provider contracting and demographic change tool box. Please select an option below to request a contract, add a provider to your existing contract or make demographic changes.
Please note: all KanCare network changes must be made through the KMMS Provider Enrollment Wizard.
Enroll as a Sunflower KanCare Provider
All participating Medicaid providers must be enrolled with KanCare and have a KMAP ID for each servicing location by January 1, 2019. Medicaid providers, participating or non-participating, will not be paid for services if billed without a valid KMAP ID.
The Kansas Modular Medicaid System (KMMS) Provider Enrollment Wizard is available for use for Kansas Medical Assistance Program (KMAP) providers. Providers can enroll in any or all the health plans – fee-for-service (FFS) and managed care organizations (MCOs) – through a single application. Please go to the Provider Enrollment System to begin this process.
Demographic Updates
If you are already a contracted provider with Sunflower Health Plan and would like to update existing information, please use our online provider update forms.
Request a New Contract (Wellcare Medicare or Ambetter Marketplace)
We appreciate your interest in Sunflower Health Plan and are excited to set up your office as a participating provider for our Wellcare Medicare or Ambetter Marketplace.

Add Provider to Existing Contract (Wellcare or Ambetter)
If you are part of an existing contracted medical or behavioral health entity, use this online contracted enrollment form to enroll a new provider.
Practitioner Rights
Practitioners have the right to obtain the status of their application at any time throughout the credentialing process and the right to review information submitted to support the credentialing application. Practitioners also have the right to correct erroneous information, should any information obtained from other sources vary substantially from the information provided with the application. Should that occur, you will be notified by the Sunflower Health Credentialing Department and will have thirty (30) days to correct the information.
