Please Encourage Our Members to Complete Annual Health Risk Screening Tool

Thank you for being a great partner to Sunflower Health Plan and our members. As we pursue avenues to engage our members, we are asking for your help.

The state requires us to collect a Health Risk Screen Tool (HRST) on all Medicaid members. This tool covers topics, like physical and mental health and social determinants of health. We use it to understand how we can serve our members most appropriately.

You can help by directing our members to our portal at https://member.sunflowerhealthplan.com. Additionally, when they complete this screen, members automatically receive a $25 reward available through our My Health Pays program.
